Menstruation 1-2 weeks early than the due day

Causes and Pathological Mechanisms

• Qi deficiencies – Spleen Qi Deficiency and Kidney Qi Deficiency
• Blood Heat – Yin Deficiency, Yang Exuberance and Liver Qi Stagnation

Chong and Ren lack of solidity, failing to control menstruation bleed, resulting in early menstruation.

Patterns

Spleen Qi Deficiency

• Symptoms: Early menstruation, or accompanied by large quantity, light colour, thin in quality, tired shen, lassitude, short of breath, indolence of speech, empty and sinking feeling in the lower abdomen (Xiao Fu), lack of appetite and loose feces.
• Tongue: Pale to with thin white coating
• Pulse: Moderate and Weak
• Treatment strategy: Strengthen spleen, tonify qi and solidify Chong to regulate menstruation
• Formula: Bu Zhong Yi Qi Tang or Gui Pi Tang
• Acupoints:

Kidney Qi Deficiency

• Symptoms: Early menstruation, small quantity, light and dull colour, thin in quality, weak and achy lower back and knees, dizziness and tinnitus, frequent urination, dim and dull face or dark patches on face.
• Tongue: Pale and dull tongue with thin white coating
• Pulse: Deep and Thready
• Treatment strategy: Strengthen kidney, tonify qi and solidify Chong to regulate menstruation
• Formula: Gu Yin Jian
• Acupoints:

Yin Deficiency with Blood Heat

• Symptoms: Early menstruation, small quantity, red colour, thick quality, malar flush, red lips, hot feeling at five centres, dry mouth and throat.
• Tongue: Red tongue with little coating
• Pulse: Thready and Rapid
• Treatment strategy: Nourish yin, clear away heat and cool blood to regulate menstruation
• Formula: Liang Di Tang

Yang Exuberance with Blood Heat

• Symptoms: Early menstruation, large quantity, red and crimson colour, thick quality, vexation with chest distension, thirsty for cold drinks, constipation, scanty and dark urine and a red face.
• Tongue: Red tongue with yellow coating
• Pulse: Slippery and Rapid
• Treatment strategy: Clear away heat, lower the fire and cool blood to regulate menstruation
• Formula: Qing Jing San

Transformation of Liver Qi Stagnation into Heat

• Symptoms: Early menstruation, small or large quantity, red and crimson colour, thick quality with clots, pre-menstrual pain and distension in breasts, chest, hypochondria and Shao Fu, vexation, irritable, bad temper, bitter mouth and dry throat.
• Tongue: Red tongue with yellow coating
• Pulse: Wiry and Rapid
• Treatment strategy: Clear away liver heat, relieve stagnation and cool blood to regulate menstruation
