Dysmenorrhea - Tong Jing

Definition

Cyclical Xiao Fu pain before, during and after menstruation, pain radiating to lower back and the sacral regions or even severe pain causing faint.

Symptoms: Cyclical dull pain in Xiao Fu before, during and after menstruation, preference for palpation, small amount of menstruation, pale colour, thin quality, dizziness and tinnitus, achy lower back and weak knees, clear and profuse urine and a dull face.
Tongue: Pale tongue with thin coating
Pulse: Deep and Thready
Treatment strategy: Tonify kidney, replenish essence, nourish blood and stop pain
Formula: Tiao Gan Tang
Acupoints:

Symptoms: Cyclical dull pain in Xiao Fu before, during and after menstruation, preference for palpation, small amount of menstruation, pale colour, thin quality, tired shen, lassitude, dizziness, palpitation, insomnia with many dreams and a pale face.
Tongue: Pale tongue with thin coating
Pulse: Thready and Weak
Treatment strategy: Tonify qi, nourish blood and harmonise the middle to stop pain

Symptoms: Cyclical distending pain in Xiao Fu before, during and after menstruation refusing palpation, chest, hypochondria and breasts distending pain, rough menstruation, dull purple colour with clots and pain eased by discharge of clots.
Tongue: Dull purple tongue with ecchymoses
Pulse: Wiry or Wiry and Uneven
Treatment strategy: Activate qi and blood and remove stasis to stop pain

Symptoms: Cyclical cold pain in Xiao Fu before, during and after menstruation, refusing palpation but eased by heat, small amount of menstruation, dull colour with clots, aversion to cold, cold limbs, blue and pale face.
Tongue: Dull tongue with white coating
Pulse: Deep and Tight
Treatment strategy: Warm meridian, dispel cold and remove stasis to stop pain

Damp Heat Accumulation

Symptoms: Cyclical hot pain in Xiao Fu before, during and after menstruation, radiating to lower back and sacral regions, or Xiao Fu pain in normal times aggravated by menstruation, large amount of menstruation, prolonged menstruation, dark red colour, thick quality and with clots, profuse leukorrhoea, yellow, thick and foul smell, low fever and dark yellow urine.
Tongue: Red tongue with yellow and greasy coating
Pulse: Slippery or Soggy and Rapid
Treatment strategy: Clear away heat, remove dampness and resolve stasis to stop pain
